New- All Grant Applicants MUST register in the System for Award Management (SAM) in order to obtain grant funding in Fiscal Year 2013. Register now at https://www.sam.gov.

 Please complete all of the application forms listed below and save them to your computer.

A complete application must include:
• SF 424 Application for Federal Assistance
• FY2013 JACS Grant Project Proposal
• “Certifications Regarding Debarment, Suspension and Other Responsibility Matters, Drug-Free Workplace Requirements and Lobbying" (DI-2010) (PDF)
• Proof of applicant's governmental, nonprofit, or institutional status
• Letter from owner and /or participating organization giving consent, if applicable


Non-Construction projects must also submit:
• SF 424A Budget Information, Non-Construction Programs

• SF 424B Assurances—Non-Construction Programs (PDF)

Construction projects must also submit:
• SF 424C Budget Information, Construction Programs

IMPORTANT! ADDITIONAL GUIDANCE ON MIGRATING CCR INFORMATION INTO SAM:
In previous grant cycles, JACS required applicants to register in the Central Contractor Registration (CCR) system, however, this system has been retired as of July 2012. All CCR information has been transferred to the new System for Award Management (SAM). If your organization previously registeredin CCR, please make sure that all organizational information successfully migrates to SAM.

To migrate your legacy system (or CCR) user account, you must first register with SAM at https://www.sam.gov. On the SAM homepage, please select “Create an Account”. Once you validate that you have access to the email address you provided during the registration process and login, you will see a message on the user dashboard (My SAM) which will ask you “Would you like to migrate a legacy system account?” Click “Yes” to begin the migration process. Alternatively, you may click on “Manage My User Roles,” then “Migrate Legacy Account” to begin the migration process. The roles you had with the CCR legacy system will be mapped to your SAM account.
